  1. Fill out this form if your ad is ALREADY DESIGNED and you are submitting it to the video team for placement in video announcements during church services.

    If your ad is NOT already designed, click HERE     and fill out a Graphics Request Form for help designing your ad. After the ad has been designed by a member of the graphics team, the designer will submit this video form on your behalf.
     

  2. Fill out this form if you need assistance in producing a video. (For instance, a skit or a video recording of your event).
