1. Fill out the form below with your contact information, number of tickets needed, a list of all ticket holders and your method of payment.

2.  To Purchase Tickets: While filling out the form below, click on the drop down menu under "PAYMENT..." and select your payment method.

3. We offer the following payment options: check, cash, Zelle, Cash App and Tithe.ly.

   1. PAYING BY CHECK/MONEY ORDER:  Fill out the form below, submit form and make check payable to "First Missionary Baptist Church." Mail check/Money order to the church's address. (see flyer above)

   2. PAYING BY CASH: Fill out form below, submit form and give cash to one of the committee members listed on the flyer above.

   3. PAYING BY ZELLE: Fill out the form below, send payment to Finance@fmbcfred.org

   4. PAYING BY CASH APP: Fill out the form below, send payment to FMBC's Cash App account: $FMBC00
